Client Centre
When you receive your insurance certificate, we will ask you to activate your group insurance file in the Client Centre. Your online insurance file gives you a number of options, such as:
Online claims
Claim online and get reimbursed within 48 hours for most care.
Dependents
Make changes to the dependents indicated in your file.
Insurance plan
Review the booklet outlining all the types of coverage offered under your group insurance plan.
Totals for tax purposes
Access your file to consult, print or save your totals for tax purposes. Easy and secure!
Healthcare professionals
Refer to the Totals for each healthcare professional under your contract.
Change of address
If you're moving, you can change your address quickly in the Client Centre.

After receiving your insurance card, show it to your pharmacist.

Mobile app
You enter your personal information one time only. You can file your prescription drug, medical and paramedical claims, and attach photos of your receipts

Direct deposit
Receive Health and Dental Care benefits faster with the direct deposit service! To enrol, simply go to the Client Centre or complete the form.
Personal Information
Please note that, in accordance with An Act respecting the Protection of Personal Information in the Private Sector, only the main participant under the contract or a person whom the participant has specifically appointed in the contract may obtain detailed information concerning the insurance contract.
The main participant fills out the Authorization for the Disclosure of Personal Information form and sends it to us. The main participant can authorize La Capitale to disclose the plan information to another person.
Modifications to group insurance
Participants wishing to make a change to their coverage (such as a change from Individual to Family coverage status) may do so without having to provide proof of eligibility, as long as Beneva receives their request within 31 days following a life event.
The employer must file the request by completing the Group insurance application or change request form and sending it to Beneva.
